Your Role:

While completing the survey, we will ask you questions exploring your on-chain actions, how you view the risks you take, and whether on-chain insurance appeals to you.

What will this Involve for You:

It will take up to 25 minutes for you to complete the survey.

You will need access to a laptop/desktop to fill out the survey.

How we will store your data:

This consent form, screener and survey data will be securely stored on Deadhead’s systems.

These files will only be accessed by the Deadhead team on this project and the relevant members of our client’s team. All files will be anonymised.

Any information collected will be kept confidential and anonymous and will be stored on our client's systems. The information you allow us to have is stored for two years. After this time all information will be deleted.

You have the right to change your mind about what we do with your information or to withdraw from the research entirely at any point and for any reason.

You can ask us to remove your information from our list and to delete the recordings and transcripts at any time by contacting: hello@deadhead.digital.

You will not be able to withdraw consent from the use of your data once an anonymous report has been created for this research project. However, you can withdraw consent for the future use of the raw data.

How we will use your data:

Any personal details will be removed. Your anonymous feedback plays a really important role in helping our client build a product that meets the needs of users.

The information we collect will ensure that our client can design and build a product that people find useful.

None of the data that we share will link back to you in any way, and we will only collect data that is relevant and necessary for this research.

Your rights:

- You do not have to take part in this research if you do not want to.

- You can ask questions at any point.

- Your feedback will be anonymous. When we share our findings, you will not be identifiable, as we will remove any personal information.

Please note that payment will only be made to respondents who provide usable data for analysis. The determination of usability will be at the sole discretion of the researcher.
